Assistance with life management skills, arranging and managing community services, receiving financial entitlement programs based on disability, and managing individual income and budget. Provides assistance and referral to meet residential/housing needs, vocational needs, health and mental health needs, socialization and self-advocacy needs, legal referral, and general counseling: weekly social activities and health and wellness programming.

Provides connections to resources such as case management services, assistance with advocacy, disability awareness training, education, recreational programs, and vocational training.

Provides consultations and assessments for care and housing needs, evaluates financial and insurance resources, household and care management, monitoring/oversight, money management, and guardian and conservator services. Facilitates communication with health care providers, provides patient accompaniment to medical appointments, and home visits available for assessments and bill paying. The Disabilities Coordination program provides service coordination, educational information, and training to families, workplaces, and other agencies and programs. Topics include but are not limited to Social Security, Retirement Benefits, Survivor/Widows Benefits, Adult/ Child Disability Benefits, Senior and Disabled Resources, Home Care Workers Infomation, and Social Security Appeals Assistance.

Provides services to older adults and adults with disabilities living in specific affordable housing complexes (Dahlke Manor, Grace Peck Terrace, Rosenbaum Plaza, Holgate House, and Unthank Plaza). Case managers assess each client’s needs and provide consultation, advocacy, and service coordination. In-home services, including housekeeping and personal care, may also be provided. Dinner is served every evening.
