Provides consultations and assessments for care and housing needs, evaluates financial and insurance resources, household and care management, monitoring/oversight, money management, and guardian and conservator services. Facilitates communication with health care providers, provides patient accompaniment to medical appointments, and home visits available for assessments and bill paying. "Just in Case" plan and estate-planning services also available.

Eligibility: Adults age 18 and older with cognitive impairments, mental health challenges, brain injury, persons with complex health conditions, those who have been subject to financial or emotional abuse of an adult or elder, and persons who need help managing financial or health-care matters.
Hours: Monday-Friday 8:30am-4:30pm or by appointment
On call 24 hours per day / 7 days per week for urgent needs of established clients

INTAKE PROCEDURE: Call for appointment
FEES: Varies by service. $105/hr for care management, $125/hr for professional fiduciary, $45-85 for various support services. Can coordinate other direct care providers.

TRAVEL/LOCATION INFORMATION: Staff will travel to meet with the client in their home. Initial evaluation fees cover travel in the Eugene/Springfield area. For hourly rate services, travel time is charged. Mileage is charged at the federal rate.
