CONGREGATE HOUSING SERVICES PROGRAM
Provides services to older adults and adults with disabilities living in specific affordable housing complexes (Dahlke Manor, Grace Peck Terrace, Rosenbaum Plaza, Holgate House, and Unthank Plaza). Case managers assess each client’s needs and provide consultation, advocacy, and service coordination. In-home services, including housekeeping and personal care, may also be provided. Dinner is served every evening.
