Provides consultations and assessments for care and housing needs, evaluates financial and insurance resources, household and care management, monitoring/oversight, money management, and guardian and conservator services. Facilitates communication with health care providers, provides patient accompaniment to medical appointments, and home visits available for assessments and bill paying. "Just in Case" plan and estate-planning services also available.
