FEES: Accepts OHP (Health Share, FamilyCare, and Open Card), Medicare, and most private insurances. Sliding scale fee available. No one is turned away due to inability to pay.
DOCUMENTS: Insurance information, if applicable. If applying for sliding scale fees, bring one of the following: last two pay stubs, previous year's tax return, or unemployment statement.
