CACOON
CaCoon (CAre COordinatiON for children) is a statewide public health nurse home visiting program that provides community-based care coordinatiON for children with special health needs. Children served in the CaCoon program have higher rates of immunization, annual well-child visits, dental care visits, and make fewer visits to the emergency department. Since family members have a central role in the care of their child, all services are planned around the desires and concerns of the family.
