NURSE FAMILY PARTNERSHIP
First-time mothers are partnered with a registered nurse early in their pregnancy and receive ongoing nurse home visits through their child’s second birthday. Nurses provide health and developmental screenings to identify potential problems with growth, hearing, social-emotional development, oral health, and other issues. Nurses also offer breastfeeding support, parenting support, and general parenting education.

Eligibility: First-time pregnant women with a household income below 185% of the Federal Poverty Level (FPL). Must enroll prior to the 28th week of pregnancy.
