Provides basic primary care. Services include women's health exams including breast exams and pap tests, chronic health conditions such as high blood pressure and diabetes, treatments for minor illnesses, and injuries (does not provide care for medical emergencies) and immunizations.

Sea Mar Community Health Center provides a class on living well with diabetes. Participants will learn how to manage their diabetes, and make lifestyle choices to live a healthier life.

Provides or connects seniors with services that allow them to remain safely in their homes and communities (elder law, home maintenance and minor repair services, housekeeping assistance, Medicare counseling, housing information and referral, advocacy, and senior ride program). Services include Oregon Project Independence Services (OPI) and community resources. Additionally offers short-term case management (options counseling) and chronic disease management.

Community Health Workers (CHWs) are community members who promote health in their own communities who work with community members to identify and resolve their most pressing health issues and eliminate health inequities. In addition to providing health promotion and chronic disease management to its clients, the Urban League addresses the social determinants of health by connecting participants to its other social service programs. Urban League has developed a CHW model to deliver services and promote healthy lifestyles.

Class series helps participants adopt the healthy habits needed to prevent Type 2 diabetes. Trained lifestyle coaches lead classes to help participants make better food choices, increase physical activity, and learn coping skills to maintain weight loss and healthy lifestyle changes. Offers a variety of programs to improve the health of adults and seniors, including group fitness classes and group sports. Also offers a diabetes prevention program and support/instruction for adults with Parkinson's and other neurological diseases. Members have open use of the swim center and fitness center.

The YMCA's Diabetes Prevention Program, part of the CDC-led National Diabetes Prevention Program, is a year-long program consisting of 25 classroom sessions. A trained Lifestyle Coach helps participants learn strategies for healthy eating, physical activity, and other lifestyle changes.
