TRANSITION AND DIVERSION
Provides long-term care case management. Staff answer questions about nursing home placement, financial and medical eligibility, health insurance issues, and complete Medicaid eligibility screenings. Specialized staff work with older adults to regain independence and move back into the community. Assists with finding facility placements or developing in-home individualized care plans. Services offered in nursing facilities include 24-hour nursing care, on-site physical rehabilitation, recovery after hospitalization for serious illness or surgery, restorative services, and end-of-life care.
