INTAKE PROCEDURE: Call for appointment
FEES: Accept most vision and medical insurance plans including Medicare, Blue Cross/Blue Shield (depending on plan), and Oregon Health Plan/Care Oregon Plan (Plus and Standard). For patients in financial need who are not covered by an insurance plan, a variety of reduced fee and other payment options are offered.
