Offers specialized, wrap-around behavioral health services to adults diagnosed with a serious mental illness. Services are customized for each individual and often include counseling, skills training, case management, peer support, and support with community navigation.

INTAKE PROCEDURE: Call to have an application mailed or faxed. Once an application is completed, applicants appearing to meet eligibility requirements will be screened to determine if the program or another in the community is the best fit for their needs. Qualifying individuals will be added to the waitlist for counseling services and contacted when a space becomes available.
FEES: Services are covered under OHP/Medicaid insurance
