Helps individuals with hepatitis C develop a care plan that will lead to stability and self-sufficiency. Also assists clients in accessing medical programs (Oregon Medical Insurance Pool or OMIP, Medicaid, Medicare, etc.) and private and government social services to meet basic needs (housing, transportation, food, alcohol and drug programs, mental health services, vocational programs, SSI/SSDI, etc.).
