Offers a range of support services to individuals experiencing intellectual and/or developmental disabilities. Services can include case management; individual and family support plans; access to brokerage services for transportation; in-home supports or residential placement; and employment and vocational services.

Hours: Monday 8am-5pm, Tuesday/Wednesday 7am-7pm, Thursday 8am-7pm, Friday 8am-noon, Saturday 9am-1pm

INTAKE PROCEDURE: Call to request an application for Developmental Disabilities services. An appointment will be scheduled with an eligibility worker who can help you through the application process.
FEES: None
DOCUMENTS: Proof of citizenship, proof of address, and records of any/all psychological and medical assessments related to the disability.
