Eligibility: Must currently live in Oregon, have a gross household income at or below 200% of the Federal Poverty Level, and have exhausted all other resources to pay for treatment. Individual Lions Clubs may have other specific guidelines; eligibility is determined by individual Lions Clubs.

INTAKE PROCEDURE: Visit website to complete and submit the "Get Help Form". No intake at physical location. Submissions are reviewed several times per week by volunteers and forwarded to a local Lions Club based on the applicant’s zip code.

Individuals without direct access to a computer can call the main phone and leave a message for a call back.
FEES: Fees vary. Currently, eye exams and hearing exams are covered by the sponsoring Lions Club. Applicants may be asked to pay up to $20 for eyeglasses and up to $175 for 1 hearing aid.
DOCUMENTS: After referral is received, applicant will need to complete a 2-page application and include recent accepted forms of proof of household income. Individual Lions Clubs may have other specific requirements for applying.
LANGUAGES: Spoken languages are English, and when available, Spanish. Spoken translations services may be available upon request. Applications are available in English and Spanish. Website translated into Chinese, French, Korean, Romanian, Spanish, & Vietnamese.
