FEES: Fees vary. Currently, eye exams and hearing exams are covered by the sponsoring Lions Club. Applicants may be asked to pay up to $20 for eyeglasses and up to $175 for 1 hearing aid.
DOCUMENTS: After referral is received, the applicant must complete a 2-page application and include recent acceptable forms of proof of household income. Individual Lions Clubs may have other specific requirements for applying.

Go past the elevators to the end of the hallway. Our office is at the very end of the hallway and is in the same suite/office as Eye Promise Optical.
