Provides referrals to local Lions Clubs, which assist people in need of eye exams, eyeglasses, hearing exams, and hearing aids. Assistance depends on eligibility and funds available at each local club. No assistance with regular contact lenses for vision correction.

FEES: Fees vary. Currently, eye exams and hearing exams are covered by the sponsoring Lions Club. Applicants may be asked to pay up to $20 for eyeglasses and up to $175 for 1 hearing aid.
DOCUMENTS: After referral is received, the applicant must complete a 2-page application and include recent acceptable forms of proof of household income. Individual Lions Clubs may have other specific requirements for applying.
LANGUAGES: English, interpretation by arrangement. Applications available in English and Spanish. Website translated to English, Chinese, French, Korean, Romanian, Spanish, and Vietnamese.

TRAVEL/LOCATION INFORMATION: If visiting in person, please use the Legacy Devers Eye Institute door located at 1040 NW 22nd Ave. Go past the Devers reception area. Turn right to go down the hallway. Go past the elevators to the end of the hallway. Our office is at the very end of the hallway and is in the same suite/office as Eye Promise Optical.
