INTAKE PROCEDURE: Call for intake appointment to be admitted as a new patient. After the intake appointment, an appointment with a doctor will be arranged. It can take several weeks or months to complete intake and be seen by a doctor.
FEES: Accepts Oregon Health Plan (Medicaid), Medicare, private insurance, and offers a sliding fee scale for uninsured and low-income patients
