Specific services include prenatal care, preventive services, immunizations, EKGs and other diagnostics, minor surgery, diabetes screenings, gynecological services, contraception, laboratory services, prescriptions and medication assistance, dental screenings, mental health evaluations, screenings for sexually transmitted infections, and referrals to specialty treatment.

INTAKE PROCEDURE: Call for intake appointment to be admitted as a new patient. After the intake appointment, an appointment with a doctor will be arranged.

FEES: Accepts Oregon Health Plan (Medicaid), Medicare, Tricare, most private insurance, and sliding scale fee payments based on income and household size
DOCUMENTS: Call for list of documents needed for intake and first doctor appointments
