FEES: Fees vary based on individual care plans. Accepts Medicaid, Medicare, VA, Blue Cross, Indian Health Services, and Tribal Funds. Financial assistance may be available.
DOCUMENTS: Must have a current alcohol and drug assessment, physical exam, and tuberculosis test prior to being reviewed and approved by the admissions committee
