INTAKE PROCEDURE: Call Fora Health main line for information or to make an appointment.

For outpatient services: Call to schedule an assessment. Appointment should last about 2.5 hours.
FEES: Varies depending on recommended outpatient treatment. Most insurance plans accepted, including Oregon Health Plan. Sliding-scale fees available to qualifying individuals. Payment payable by OHP, private insurance, fee for service, Washington County-funded outpatient slot.
DOCUMENTS: Insurance information, if applicable. Proof of income if applying for sliding scale fees.
