FAMILY CAREGIVER SUPPORT PROGRAM
Provides unpaid family caregivers with support, information, referrals, and limited funding for respite care. Educational classes are also provided to assist family caregivers in caring for themselves or someone else. Support groups for caregivers are offered at various locations. Also provides management and delivery of the Chronic Disease Self Management Program classes: Living Well With Chronic Conditions and Living Well with Diabetes.

Eligibility: Adult unpaid family members and other individuals providing in-home care for a person age 60 or older, or to someone of any age with a diagnosis of dementia/Alzheimer's or other organic brain disease.

Grandparents and relative caregivers (step-grandparents or relatives by blood, adoption, or marriage) age 55 and older who are the primary caregivers to a child age 17 or younger. The child may be over age 17 if they have a disability and are financially dependent on the relative/grandparent.
Hours: Office hours: Monday-Thursday 8am-6pm. Educational program times vary.
