Provides a wide range of primary and preventive health care services to the community. Services may include: acute care treatment, general medical exams, women's annual exam, STD screening and treatment, pediatric care, vaccinations, mental health evaluations and counseling services, and drug and alcohol assessment. Also offers assistance enrolling and re-enrolling in OHP.

INTAKE PROCEDURE: Call for information and to schedule an appointment
FEES: Accepts OHP, Medicare and most private insurances. Sliding scale fees and payment plans available to individuals with high deductibles or no insurance.
DOCUMENTS: Insurance information, if applicable. Proof of income for those applying for the sliding scale fees.
