COMMUNITY DEVELOPMENTAL DISABILITIES PROGRAM
Offers support services to individuals whom experience intellectual and/or developmental disabilities. Services can include case management, individual and family support plans, access to brokerage services, in-home supports or residential placement, employment and vocational services, abuse investigation, and protective services.

Eligibility: Individuals with an intellectual or developmental disability. Not all persons who exhibit disabilities are eligible for services. Test results and diagnosis have specific criterias.

If denied eligibility, you may request a hearing. The process is described in the determination notification letter.
