COMMUNITY DEVELOPMENTAL DISABILITIES PROGRAM
Offers a range of support services to individuals experiencing intellectual and/or developmental disabilities. Services can include case management; individual and family support plans; access to brokerage services; in-home supports or residential placement; employment and vocational services; and abuse investigation and protective services.

Eligibility: Children and adults with an intellectual or developmental disability. Not all persons who exhibit disabilities are eligible for services. Test results and diagnosis have specific criteria that must be met. An eligibility team will review all records to determine if the requirements are met. Applicant must be an Oregon resident and U.S. citizen. If you are not satisfied with the eligibility determination, you may request a hearing. The process to request this is described in your notification letter.

INTAKE PROCEDURE: Call to book an intake appointment with Klamath County/Developmental Disability Program. An Eligibility Specialist can guide you through the application process.

DOCUMENTS: Proof of citizenship, proof of address, and records of any/all psychological and medical assessments related to the disability.
