DEVELOPMENTAL DIVERSITY (DISABILITIES) PROGRAM
Offers a range of support services to individuals experiencing intellectual and/or developmental disabilities. Services can include case management; individual and family support plans; access to brokerage services for transportation; in-home supports or residential placement; and employment and vocational services.

Eligibility: Children and adults with an intellectual or developmental disability. Applicant must be an Oregon resident and U.S. citizen.

INTAKE PROCEDURE: Call to request an application for Developmental Disabilities services. An appointment will be scheduled with an eligibility worker who can help you through the application process.
FEES: None
DOCUMENTS: Proof of citizenship, proof of address, and records of any/all psychological and medical assessments related to the disability.
