SENIORS MENTAL HEALTH
Mental health consultation, evaluation, treatment, and case management for seniors in care facilities and referred from private medical practitioners. Enhanced care outreach services include intensive case management, counseling, and behavioral interventions for senior and physically disabled adults with serious and persistent mental illness.

Eligibility: Adults 65 and older and people under age 65 who are homebound due to a physical disability
Hours: Monday-Friday 8am-5pm
Some services available evenings and weekends by arrangement.
