THE FOUNDERS CLINIC
Clinic provides primary medical services for individuals without access to health care. Volunteer medical professionals provide routine preventative and non-emergency medical care. For chronic conditions such as hypertension, diabetes, asthma, and heart disease, care is provided in addition to education and equipment for self-care. Eye exams and glasses are also available. More specialized services that cannot be provided on-site are referred to a network of specialty physicians in the region.

Eligibility: Age 18 and older with income 250% of the federal poverty guidelines for the last 3 months, with no health insurance or access to health care, gap insurance/medical care needed temporarily, underinsured (cannot afford care for chronic conditions under current health care plan).
Hours: Health Clinic: Tuesday/Wednesday 1pm-5pm, Thursday 5pm-9pm

INTAKE PROCEDURE: Call for appointment or eligibility screening
FEES: None
DOCUMENTS: Proof of residency. Must also show proof of (any type of) income for the last 90 days. If homeless or undocumented, some types of alternative documentation may be accepted. Eligibility screeners will meet with prospective clients and assist them in providing the requested documentation.
