Services provided include, but are not limited to information and referral, case management, eligibility determination for Medicaid and SNAP (food stamps), Adult Protective Services, licensing of adult foster care homes, private and pre-admission nursing home assessments, Senior Health Insurance Benefits Assistance (SHIBA), Senior Peer Mentoring, respite care, options counseling, Oregon Money Management, and health and wellness programs.

Go four miles; a business complex will be on the left. Turn left at the first entrance into the complex and go between two buildings and over a speed bump. It will be the next building on the left, a one-story beige building.
