FEES: Adapt is a preferred provider for most major insurance plans. Offers a sliding fee discount based on family/household size and applicable income following Federal Poverty Guidelines. No one will be denied care for inability to pay.
DOCUMENTS: Insurance information, if applicable. Proof of income if applying for sliding scale fees.
