Works with 18 churches to distribute food each weekday and assists financially with several emergency needs including medical, dental, and prescription bills, utility bills, propane and firewood needed for heating and cooking, work clothing, immunizations, and glasses (no contact lenses assistance available). All financial assistance is provided to the vendor on behalf of the client.

Provides prepared food boxes and hygiene items. Households may access food boxes once per month. Also helps with propane and prescription assistance. One-time-only prescription assistance is offered for most types of medications, up to $100 maximum. Payments are made directly to a cooperating pharmacy. Households may fill a 5-gallon propane tank once every 2 months.

FISH of Lebanon provides rent assistance, utility assistance, heating fuel assistance, prescription assistance, and clothing. Also offers financial assistance to obtain a valid ID card or drivers' license through the Oregon Department of Motor Vehicles (DMV). Assistance provided once per year for each category of service.

Oregon Family to Family Health information Center (OR F2F HIC) operates a call-in center for family members of children and youth with special needs and chronic health conditions (developmental and emotional disabilities, behavioral problems, and chronic physical illnesses) and professionals who serve them. The center specializes in providing information and resources related to health information, health care access, and special needs systems and supports. All calls are confidential.

Families may also request to be placed on an email or postal mailing list to receive regular correspondence about issues of importance to children and youth with special needs in Oregon.

Helps Oregonians purchase prescription drugs at up to an 80% discount. Website maintains list of participating pharmacies and current drug prices.

The Co-Pay Relief Program (CPR) provides direct financial assistance for specific diseases/diagnoses to financially and medically qualified patients for co-payments, co-insurance, and deductibles required by a patient’s insurer for pharmaceutical treatments and/or prescription medications prescribed to treat and/or manage their disease. CPR does not make payments for pharmaceutical treatments and/or prescription medications relative to the patient's diagnosis that are not covered at all by the patient's health insurance.

Polk County Resource Center provides the following services: Emergency financial assistance for rent, utilities, birth certificates, ID cards, and prescriptions; hygiene items, diapers, transportation passes, shower passes and gas cards; comprehensive information and referral; child care resources and referrals and education to providers and parents. Also offers seasonal programs including school supplies and holiday assistance.

The Low Income Subsidy for Medicare Prescription Drug Coverage, also known as the Extra Help program, is a prescription assistance program available under Medicare prescription drug coverage (Part D). Medicare beneficiaries with limited resources can receive assistance with prescription costs, such as monthly premiums, annual deductibles, and prescription co-payments.

May be able to help with rent, utilities, prescriptions, identification & birth certificates, gas, regional bus fare, and other urgent needs. Usually only able to pay a portion of the requested amount.

The Early Intervention Program (EIP) provides help to eligible persons with HIV to get medical care and support to improve their health. This can include help with medical appointments and laboratory tests, dental care, HIV drugs and health insurance premiums.
