Provides routine services, such as cleanings, fluoride treatments, sealants, and fillings, as well as urgent dental care. No oral surgery, periodontics, or orthodontics.

Some dentures provided if patient's oral health meets certain criteria.

INTAKE PROCEDURE: New patients: Call the new patient line
Established patients: Call the clinic to make an appointment
FEES: The following OHP dental plans are accepted and will be billed: CareOregon Dental, Capitol Dental, and Delta Dental. For patients without insurance, there is a $25 fee due at appointment check-in, with the remaining balance due at check-out. Sliding scale fees available by appointment only; NO FREE SERVICES.
DOCUMENTS: Insurance information, if applicable. Proof of income if applying for sliding scale fees.
