INTAKE PROCEDURE: Must have either a case manager with Jackson County Developmental Disabilities Services, a personal agent with Creative Supports, Inc., or a professional in the community (doctor, nurse, social worker, etc.) who can verify an intellectual/developmental disability diagnosis and fill out an application on behalf of the individual.

DOCUMENTS: Proof of emergency situation such as past-due notice on utility bill, shut-off notice, letter from landlord, or eviction notice
