BIRTH CERTIFICATES
As of July 1, 2018, individuals who are homeless can visit a local homeless service provider or organization to get help ordering their Oregon birth certificate. Participating agencies can provide help with completing the birth record order form, provide infomation on documentation needed, provide a check for the payment, and prepare the affidavit for “Homeless Status for Fee-Exempt Certified Copy of Birth Certificate” to include with your order.
