COMMUNITY HEALTH SERVICES
The First Steps program provides Maternity Support Services and Infant Case Management. A Care Team consists of a Nurse, Social Worker, Case Manager, or Dietitian that will see the client based on their needs throughout the pregnancy and two months postpartum. The infant can be seen through the baby’s first year. The Care Team will assist the family with resources, education regarding pregnancy, breastfeeding, labor & delivery, parenting skills, or setting goals. Services are offered by phone, in the clinics, or in the client’s home
