INTAKE PROCEDURE: Call for appointment. If voicemail is full, call back. Cannot guarantee same-day or same-week appointments. May accept some walk-ins but walk-in clients are not guaranteed a same-day appointment.
FEES: Sliding scale fees based on income and family size for patients with no health insurance. $25 minimum fee. No one is turned away for inability to pay.

Insurance accepted: CareOregon, Family Care, Oregon Health Plan (OHP Open Card), Healthy Kids, Lifewise, Moda Health, PacificSource, United Healthcare Medicare, and Providence.
DOCUMENTS: Insurance information if applicable. Proof of income if applying for sliding scale fees.
