INTAKE PROCEDURE: Call for information or appointment
FEES: Accepts OHP, Medicare, and most private insurances. Sliding scale fees based on income and family size for patients with no health insurance. $25 minimum fee. No one is turned away for inability to pay.

Insurance accepted: CareOregon, Family Care, Oregon Health Plan (OHP Open Card), Healthy Kids, Lifewise, Moda Health, United Healthcare Medicare, Providence Medicare and Private, and PacificSource.
