ADULT DAY SERVICES
Operates two state-certified adult day centers providing care for older adults, adults with disabilities (intellectual, cognitive, and/or physical) and support for family caregivers. Adult Day Services provide a structured program designed to meet the needs of functionally and/or cognitively impaired adults through individually planned service, supervision, social and related support services, and health monitoring in a protective setting during any part of a day, but less than 24-hour care.

INTAKE PROCEDURE: Call for information. Complete and submit participant intake packet by filing online or call to have form mailed to you.
FEES: Center maintains contracts with Medicaid, the VA, and Oregon Project Independence (OPI). Fees are $95/day.

SITE HOURS: Monday, Wednesday, and Friday 8am-2pm
