Primary Care clinic provides diagnosis and treatment for acute and chronic medical care, such as infections and diabetes. Services also include general physical exams, pregnancy testing, STI testing, and abscess and other common illness treatments. Limited urgent care, mental, and behavioral health services available. Outside In's chiropractic and acupuncture treatment, as well as a full Chinese medicine dispensary is temporarily closed.

Hours: Monday/Thursday/Friday 8:30am-5pm, Tuesday/Wednesday 8:30am-7pm. Closed until 11 am on the second Tuesday of the month.

INTAKE PROCEDURE: Walk in or call to schedule an appointment. Walk-in clients may have to wait long periods. Same-day appointments are limited.
FEES: None for individuals under age 21. Sliding scale fees are available to those under 200% of the Federal Poverty Level. No one is turned away for inability to pay. Accepts Medicare, OHP Open Card, HealthShare (CareOregon, Providence), and most private insurance. Insurance will be billed, if applicable.
DOCUMENTS: Insurance information, if applicable. Proof of income if applying for sliding scale fees.
