PARTNERSHIP PROJECT/MEDICAL CASE MANAGEMENT
Partnership Project provides comprehensive client-centered HIV case management services to those living with HIV; and to the family members who constitute the patient's major support system. Nurse, Medical Case Managers, Insurance Assisters and Medical Navigators assure that eligible HIV positive individuals have access to health insurance and other allied health and support services that assist them in maintaining focus with on-going health and medical care regime.
