COVID-19 INFORMATION: Due to COVID-19, all appointments are over the phone or video
INTAKE PROCEDURE: Call for initial intake, which is a 15-20minute interview. Waitlist is currently 8-10 weeks to be scheduled with a counselor
FEES: Accepts most private insurance (no OHP/Medicaid or Medicare). Sliding scale fee available is for uninsured patients.
DOCUMENTS: Insurance information, if applicable. Proof of income if applying for sliding scale fees.
