-Primary care for chronic medical issues
-Blood tests (completed off-site)
-Gynecology, Pap tests and breast exams
-Referrals to specialist through Project Access

New Heights Clinic is UNABLE to see patients for
-Immunizations
-Narcotic prescriptions
-Prenatal care
-Urgent needs
-Tuberculosis tests
-Physicals for Workman's Comp, Department of Transportation, Employment, School, SSI, or Disability

Eligibility: Individuals without medical insurance (are not eligible for Apple Health) who are under 65 years old and whose income is below three times the Federal Poverty Guidelines:
$3,645 monthly income for household of 1
$4,930 monthly income for household of 2
$6,215 monthly income for household of 3
$7,500 monthly income for household of 4
$8,785 monthly income for household of 5
Hours: Monday-Wednesday 9am-11am and 1pm-4pm, Thursday 9am-11am,1pm-5pm

INTAKE PROCEDURE: Call for appointment. Appointments must be confirmed by noon the day before, or the appointment will be rescheduled. Monday appointments need to be confirmed by Thursday at noon.
