COLLABORATIVE HOME SERVICES
The Collaborative Home Services (CHS) Support Specialist provides home-based services to families to enhance family stability and support workforce readiness. Program recipients will receive a full assessment, an individualized goal plan, relevant resources to support plan progress, and other tools to support self-sufficiency.

Services may include:
-Assistance with employment readiness, including interviewing clothes
-Assistance with applying for Social Security Disability, Earned Income Credit, Vocational Rehabilitation, or FAFSA (Free Application for Student Aid)
-Access to medical/dental/mental health treatment for self and family
-Financial literacy
-Education planning and connections for self and/or children
-Effective communication and conflict resolution skills to improve job retention
-Housing assistance
-Time management
-Development of informal support system
-Minimal financial assistance

Eligibility: Must have a child 5th grade or younger in the TANF group OR youngest in TANF group is 17-18 years old OR have fewer than 2 years left of TANF before the 5-year limit. Eligible clients must also have barriers to self-sufficiency, such as mental health, substance abuse, low/limited education, physical health, child truancy, or other challenges.

COVID-19 INFORMATION: Due to COVID-19, services offered in-person, via phone, email, or telehealth conference
INTAKE PROCEDURE: Must be referred by DHS Self-Sufficiency worker. There is an initial appointment to go over program details and sign paperwork, followed by a second appointment with a full assessment to identify strengths and barriers of participants.

DOCUMENTS: CFS referral form completed and turned in to DHS Self-Sufficiency worker
