COMMUNITY SUPPORT SERVICES
Provides a continuum of care for people with mental health challenges, designed to help consumers move toward maximum recovery and successful community life. Services include individual and group therapy, skill development, targeted case management, hospital discharge services, dual diagnosis treatment, peer-delivered services, representative payee services, residential and 20-hour personal care services, and coordinated care services. Most services are provided at client's residence.

COVID-19 INFORMATION: Due to the Covid-19 crisis, clients can call main line to schedule appointments and will be assisted in setting up a telehealth (video) or telephone appointment.
INTAKE PROCEDURE: Must be referred by a state or regional mental health program or via assessment process. Call for information and to discuss needs with a counselor.
FEES: Varies by service. Sliding scale fees available. Assistance to review income and insurance coverage is available.
