INTAKE PROCEDURE: -Call office main line for information or to make an appointment
-Call intake for Medications to Treat Addiction (MAT)
-Outpatient assessments: Walk in; appointment should last about 2.5 hours
FEES: -Varies depending on outpatient treatment.
-Most insurance plans accepted, including Oregon Health Plan
-Sliding-scale fees available to qualifying individuals
-Payment payable by OHP, private insurance, fee for service, or Washington County funded outpatient slot
DOCUMENTS: Insurance information, if applicable. Proof of income if applying for sliding scale fees.
