SERVICE NAVIGATION (CASE MANAGEMENT)
Provides collaborative, intentional approach focused on navigating barriers with participants to meet their goals and create a support system through building community. Provides a strengths-based, trauma-informed care and harm reduction approach. Also offers document recovery, Tribal Enrollment, Public Benefits Enrollment/Navigation, Housing, School (Re-enrollment/College), Harm Reduction/Survival/Hygiene Supplies, Advocacy, Systems navigation, and Direct Client assistance as funding allows (rent support, move-in assistance, relocation, document recovery, etc.)
