SERVICE NAVIGATION (CASE MANAGEMENT)
Provides a collaborative, intentional approach focused on navigating barriers with participants to meet their goals and create a support system through building community. Provides a strengths-based, trauma-informed care and harm reduction approach. Also offers document recovery, tribal enrollment, public benefits enrollment/navigation, housing, school (re-enrollment/college), harm reduction/survival/hygiene supplies, advocacy, systems navigation, and direct client assistance as funding allows (rent support, move-in assistance, relocation, document recovery, etc.)
