INTAKE PROCEDURE: Call to schedule an appointment.
FEES: Accepts OHP and some private insurance. Sliding scale fees available for uninsured and underinsured patients. Minimum $20 fee due at time of medical appointment for uninsured patients. No one is refused care due to an inability to pay.
