Health care services include: primary care, walk-in care, hepatitis C treatment, mental health treatment, acupuncture, and an on-site pharmacy. Most addiction treatment services including suboxone (no methadone treatments). Transitional, supportive, and permanent housing services also available.

INTAKE PROCEDURE: Walk in or call for appointment. Assistance registering for insurance from Oregon Health Plan or Medicare is provided if qualified.
FEES: Accepts Oregon Health Plan (including CareOregon and other Medicaid), Medicare, self-pay (discounts available for patients living on low incomes), some private insurance. No patient is turned away for lack of insurance or inability to pay.
DOCUMENTS: Insurance information, if applicable. Proof of income if applying for sliding scale fees.
