COORDINATED HOUSING ACCESS TEAM (CHAT)
The Coordinated Housing Access Team (CHAT) is a partnership of agencies working with homeless individuals and adult family units to connect them with permanent supportive housing-related resources. The CHAT team conducts a Service Prioritization Determination Assistance Tool assessment (SPDAT) to determine the risk of a person or family unit's situation and their prioritization of services. At the time of assessment, shelter and transitional housing information will be provided. Does NOT provide immediate services.

Eligibility: Must meet the following:
-Households with at least one adult age 18 and older, with no minor dependents
-Must be experiencing homelessness (couch-surfing, in a motel, on the streets/in a vehicle, in a shelter/transitional housing, or in some other place not intended for habitation)
-Households currently in housing that are on a lease and facing eviction do not qualify
-Households with minors age 17 and younger do not qualify
Hours: 24 hours per day / 7 days per week
