INTAKE PROCEDURE: Call to schedule a ride up to 30 days in advance. Next-day rides must be scheduled by 4pm the day before. Trips longer than 70 miles need 2 business days' notice.

DOCUMENTS: Name, phone number, date of birth, date and time of appointment, pickup and dropoff address and phone number, medical provider's name, and person's name you will be seeing at the appointment
