INTAKE PROCEDURE: Call for appointment when the line is open and leave a message with your first and last name and phone number. Calls are returned on a first-come, first-served basis. No walk-ins accepted.

DOCUMENTS: Photo ID for adults, Social Security cards, and proof of income for the past 30 days for all adults
