COVID-19 INFORMATION: Due to COVID-19, offices are closed to the public. Services are available remotely, including interpretation and translation services. Contact the agency for additional information.
INTAKE PROCEDURE: Call for appointment when the line is open. Leave a message with your name, phone number, Social Security number, and utility company name to request an appointment. Please speak clearly and slowly.

DOCUMENTS: Must bring all of the following:
-Current utility bill in applicant's name
-Social Security cards for all household members
-Photo ID for all adults in the household
-Proof of income for the past 30 days for all adults in the household (TANF, SSI, unemployment, pay stubs, pension, child support, etc.). If any adult in the household has no income, they must come to the appointment with the applicant.
