INTAKE PROCEDURE: New patients: Call the new patient line
Established patients: Call the clinic to make an appointment
FEES: The following OHP dental plans are accepted and will be billed: CareOregon Dental, Capitol Dental, and Delta Dental. For patients without insurance, there is a $25 fee due at appointment check-in, with the remaining balance due at check-out. Sliding scale fees available; no free services.
DOCUMENTS: Insurance information, if applicable. Proof of income if applying for sliding scale fees.
