CO-PAY RELIEF DIVISION
The Co-Pay Relief Program (CPR) provides direct financial assistance for specific diseases/diagnoses to financially and medically qualified patients for co-payments, co-insurance, and deductibles required by a patient’s insurer for pharmaceutical treatments and/or prescription medications prescribed to treat and/or manage their disease. CPR does not make payments for pharmaceutical treatments and/or prescription medications relative to the patient's diagnosis that are not covered at all by the patient's health insurance.

Eligibility: Insured individuals with prescription coverage and income below 400% of the Federal Poverty Level (FPL). Covered diseases/diagnoses:
-Various types of cancer (bladder, breast, cervical, non-small cell lung, ovarian)
-Various types of metastatic cancer (breast, colorectal, gastric, prostate)
-Chronic pain
-COVID-19
-Electrolyte imbalance
-Gaucher Disease
-Hepatitis B and C
-HIV and AIDS
-Homozygous familial hypercholesterolemia
-Inherited or acquired lipodystrophy
-Multiple myeloma
-Multiple sclerosis
-Myelodysplastic syndrome
-Osteoporosis
-Periodic paralysis
-Pulmonary Fibrosis
-Renal cell carcinoma
-Spinal Muscular Atrophy

Also serves those insured through federally administered health plans like Medicare, Tricare, and Medicaid.
